Quite a few. Two of them I found out recently (yesterday only) are Be2 - Escape from Pongland (easily the best hidden gem I've ever come across. Ever.) and Spirit. Though spirit might not be a hidden game considering its 500+ posts thread here, but for me it was hidden as I never thought I'll enjoy this game. Kept on ignoring it for the longest of the time but the moment I tried the game's pulse mode, I was hooked.

Although it might not be too hidden, I suggest Cat-in-a-Box's "Red Conquest!". It didn't get anywhere near the sales it should have. I think it was the learning curve that turned everyone off. It's really easy to master if you just try a bit. And I second what IMNS said about Spirit. Lovely game you've got there.
Iced in - As a puzzler, it ranks right up there with trainyard for me. Some levels are very challenging Stick Golf - Great casual game with a lot of challenge. Babylonian Twins - People know about it, but don't think it ever quite took off Soul - It's the game with the best atmosphere that I have played Miner Disturbance - Fun not-too-challenging puzzler Gavity runner - Not sure if it can be classified as hidden, but easy gameplay and if you are aiming for gold in all levels, you need tons of patience.
Sky Dragon was an awesome JShmup long before Cave hit the scene. Unfortunately, it no longer seems to be available on the app store. Dragon Force Lite was another incredible shmup. Unfortunately, the full version never materialized! I remember when Defender Chronicles was a hidden gem...
Double Mind- it's a pretty creative take on mastermind that was well done. Critical Wave- innovative arena shooter (beware of flashing lights ingame though) Not mine but someone else's- Hoggy. Pretty good underknown game.
Indie Games with Good Reviews (but not enough attention): -Eliss: A frantic puzzle-action-shape game -Beat It! - A whole new take on the rhythm game... requires SKILL! -Enviro-Bear 2010 : A ridiculous bear driving simulator. -Snail Mail: Smooth playing time trial type racer. -Germs : The same idea as Osmos except with cooler physics and graphics! -Magnetic Shaving Derby: A weird retro styled shaving game with a lot of charm ...Yeah. Check them all out.
Haha oh man I agree about Snail Mail. It's the very first game I got for my iPhone way back when it was first released. I got it the very first day and it has never left my iPhone since. I got it for the 2G and it still has all the data from it on my 3GS.
Land Air Sea Warfare - My fav RTS. Best rts in app store Speed Forge Extreme - Racing shooter. Still in my top 3 games. Hard to define what is exactly 'hidden', but here is a list of some I own that haven't exactly seen commercial success, but might have a thread of reasonable length here: Above - a running/endless climb high score game with really pretty graphics that got totally eclipsed by the launch of the iPad. Infinity project - a bang for your buck first person shooter with reasonable difficulty and a good length.
Skeeball is nowhere near hidden. It's been a constant Top 25 (or Top 50 when there are sales) since who knows when.
Sorry if some of these are repeats, but I don't want to read through 8 pages of posts. Ninja Jumper: Fun endless game-play, with achievements. Star Pagga: A space-combat game, like Star Battalion, with multi-player, DLC, and a long-ish campaign it's fun to play alone and with friends. Twin Blades: Side-scrolling hack-n-slash zombie game, very fun to kill zombies with a giant scythe. Same creators as Pix'n Love rush. Fastar!: Another side-scroller, it's an RPG battle against squares of various sizes. Doodle Pool: A really good pool game with a doodle theme. Xeno Wars: Cartoon Wars meets match-3, a unique concept done well. Soosiz: Super Mario Brothers 3 meets Super Mario Galaxy. Trace: A draw-your-own-platforms get-to-the-exit puzzle-platformer. *FREE* Gomi: Great game, Soosiz meets I love Katamari, very long game with excellent replay value, made by the same developers as Trace. Vector Tanks: A kill-all-the-enemies first person tank shooter, with a cool neon art style. I could go on.
2xl rally is an amazing gem no one kows about but it's as good and physics as realistic as any of the Colin McRae games. Also golden minder or gold miner isn a great platformer once u change the controls in options.
ChronoSgear is definitely one. Game Dev Story. I'm not sure if it counts, but Carnivores: Dinosaur Hunter is awesome. It's high caliber, though.
