If you're using a mac, just open the attachment in Preview or Adobe Acrobat, type into the two fields at the bottom, and then make sure you "save as" a .pdf file, which should be the default anyway.

Ok, thanks. What I did (i don't know if this is right, but I just opened up the .pdf in a preview and then typed in the fields then hit Command + S, then replyed to the original message, maybe I should try it again your way then send the new .pdf. Oh well, I'll just wait and see if I get an email back.
