I have only ever got it during the infect phase, that is before no more healthy pop up. Also I got mine in normal disease, not using cheats, I could be wrong on that being a requirement, probably am. I assume you have tried both ways in my post on page 57?
I think: Air 1, livestock 1, air 2, livestock 2, rodent 1, drugs 1+2, cold 1+2 then swine flu. Wait.... Insanity, paralyse. Nuke the hell out of them

A step by step, for contaminated and China Nuke. Or a step by step for contaminated and advice on reducing threat pop up randomness. If I can reliably, one in every two games would be nice, get the pop up, I can tweak my strategy to try and get China, as I already have Russia.
Okay, I took what you did and annotated it. The root, I believe, of your problems comes from the very beginning of the game. Your very first actions should always be Drug 1&2. Then get coughing. Devolve anything else that comes along. Then Wait. Let it spread a little bit. Then as you gain more DNA, add Livestock 1&2. Then wait again. Let it get to the USA. Once it's there, wait some more. You need DNA. Once you have around 40-50 (which should not take long), get the following all at once (this seems to help for me): coughing, sneezing, nausea, vomit, insomnia, paranoia, fever, diarrhea, pneumonia. Then wait again. Let the USA get more infected, and rack up more DNA. You do not need to spend them on transmission. Not to get the POTUS sick. You only need symptoms. It may or may not take a while to get him sick, but just sit tight, and eventually he will. Just don't spend DNA trying to get it to happen faster. If seizures mutates, just let it go. Do not devolve. Same with inflamation. Once he is sick, evolve insanity. You should have plenty of DNA to do this. Wait. EVENTUALLY, Spalin will get elected. When she does. WAIT. Make sure lethality is low, and severity is medium. Devolve all other symptoms except the ones noted above. Are you following me so far? Any questions?
Not to derail the thread about nukes and space, but could somebody give me some hints about how to defeat the new plague? I'm having a lot of trouble.
For the record, this was blind luck Blind luck, I was trying a nuke attempt Immunity cheat, start in USA, blood 1+2, air 1+2, water 1+2, extreme bio, drug 1+2. Following ACE42292's post on page 58, symptoms were insomnia, paranoia, rash, sweating, skin lesions, anaemia, haemophilia, plus internal hem. Internal hem has nearly the same lethality as fever, diarrhoea and inflame from swine flu. POTUS ill so added seizures and insanity. My usual nothing happening for a long while At 91% cure I got the space mission planned, so I devolved every symptom except insomnia, paranoia and insanity, so astronauts would appear to be healthy whilst still allowing me to complete the nuke if it happened. At 93% cure space mission launched, and shortly thereafter got Spalin voted in, so added inflame. Didn't have enough for paralysis if she threaten so started collecting more DNA bubbles. Some months later space cure failure message. I never got the threat pop up
I have gotten Palin to threaten Russia consistently. Once she is voted in I immediately devolve Insanity. Short time later she threatens and I evolve Paralysis. Very shortly after that, Goodby Comrades!
Contaminated Package Ahh, feels good to have at least one achievement completely figured out. I can't find any randomness in this one at all.
