No energy system I've encountered yet. You can replay/fail a level as much as you want. There are timers on your "helpers", which can give you a boost in a surgery. Every completed surgery awards gold, which you can use to buy card packs a la Clash Royale. These are used to upgrade your tools (5% extra speed/efficiency/etc). I earned enough gold every ~2 surgeries to buy a pack. Will have to see if things change on later levels. Was pleasantly surprised there were no timers, other than the optional surgeon helpers. BUT. I was wrong. After you progress a handful of levels, you reach one that WON'T let you progress until one of your helpers is out of cooldown. Even if you want to attempt it without them - your only option is to spend gems or to wait several HOURS for them to become available. Same for all the special modes for repeating a level, and the special missions you can rank on leaderboards for. You must have a helper to even attempt the level. Very frustrating, ruins an otherwise great game. I was able to do a 5x pull with free gems, and I now have 7 assistants. They have cooldowns from 2-12 hours, and are required for bosses/special levels. There are 6 bosses in the first 35 levels, so it's not super terrible. Each level also has a sudden death/partner special that requires any partner (sudden death) or a specific partner (special), but they aren't required for progression. From what I've been told by Adult Swim, they didn't have the means to keep the previous games updated for newer hardware and software versions, and Amateur Surgeon 4 contains so much of the content from the previous games it made the older games redundant. It's sort of like an Amateur Surgeon Remastered.
